In December 2018, we will be relocating our Grand Health Center to our San Marcos location. The location change will provide patients with access to all that the San Marcos Health Center has to offer which includes, but is not limited to, Dental, Behavioral Health, Pediatrics, Women’s Health, WIC, Billing services, Community Programs and our on-site Pharmacy.

Patients will continue to receive excellent care from their existing provider – we will help to make your transition as easy as possible. Our staff are on stand-by for questions or concerns.

What does this relocation mean to me?
Relocating to San Marcos Health Center (SMHC) means that you and your family will have access to all that the SMHC has to offer which includes, but is not limited to, Dental, Behavioral Health, Pediatrics, Women’s Health, Family Medicine (adults and pediatrics) WIC, Billing Services, Community Programs and our on-site Pharmacy.

When is the clinic relocating?
All Grand Ave. Family Medicine patients and staff will be relocating to the SMHC location in December 2018.

What is the address of the SMHC?
All Grand Ave. Family Medicine patients and staff will be relocating to our newly remodeled facilities at 150 Valpreda Road, right off of Rancheros Dr. in San Marcos.

Where do I park when I arrive?
There is parking available in front of the health center as well as parking along Valpreda Rd.

Will I need to see a new provider?
No. You can continue to see the same provider at the SMHC.

Do I need to make any changes to my insurance?
No. You do not need to make changes to your insurance. NCHS is here for you to make the transition as easy as possible.

Can I still access my health records if the clinic is relocating?
Yes. You can access your health records by contacting NCHS at 760-736-6767 for more information.

How will I know where to go for my next appointment?
NCHS is here to answer any questions you may have. When you arrive at the SMHC, signage will help you to easily find your way to your appointment. You can also visit our Information Desk located at the entrance of the health center.
