What is San Diego Health Connect?
Answer: San Diego Health Connect (SDHC) is our community’s secure electronic access to your current medical information. SDHC allows hospitals and medical offices throughout San Diego County to securely and timely share important health information. This allows for better communication between doctors to have complete and current health information to provide you with the best care possible. Participating in SDHC will also help prevent duplication of tests and procedures. All NCHS patients are automatically enrolled in this service.
What is the opt-out form?
Answer: If you do not want to participate in San Diego Health Connect, you will need to complete an opt-out form. This form must be completed with a signature in order to process your request. Please ask the Front Desk for a copy of the form, or you may call our Health Information Department at (760) 736-6717 to have a form mailed out to you. It may take up to 30 days to complete your request.

How will my privacy be protected?
Answer: Your privacy is extremely important. Only authorized members of your healthcare team can view your health record. San Diego Health Connect has created a secure system for information exchange that complies with state and federal privacy laws to make sure your health record is safe.
How will San Diego Health Connect benefit me?
Answer: Today, most medical records are not shared. If you need care from different healthcare organizations, your records must be sent either through mail or fax. Sometimes, records aren’t sent at all, or important pieces of information get lost in transit. When your doctors are able to communicate with each other and share your health information, they can provide you with the best care possible.
What is this going to cost me?
Answer: Participating in San Diego Health Connect is free to all patients.
