HIV/AIDS Community Case Management Program (CCMP)
Project Summary
NCHS’ Community Case Management Program (CCMP) assists persons infected with AIDS and HIV; the programprovides medical and dental services to program patients as well as HIV testing, counseling and outreach services. NCHS' Community Case Management is the second largest program in San Diego County and is funded by the Ryan White HIV/AIDS Treatment Modernization Act of 2006, Parts A, C and D, State HIV Case Management Program, Medi-Cal and private funds. Our mission is to “enrich the quality of life of those infected or affected by HIV/AIDS and those at risk in the community, by providing innovative services from experienced and caring staff, in a sensitive and fiscally responsible manner.”
The Coordinated Services Center - Enhancing and enriching physical health and emotional well-being.
The Coordinated Services Center (CSC) is a “one stop shopping” concept for the delivery of services to the HIV community in North County where clients can go to one location and access or get referrals to all the major services they may need. The CSC provides information and referral, peer advocacy, HIV testing, case management, ADAP enrollment, bi-lingual services, access to primary medical care, volunteer opportunities, social events, HIV educational presentations and collaborative services such as mental health counseling, women and children advocacy, legal services and support groups.
The Coordinated Services Center is a drop-in center where people meet to access services and interact with each other in a variety of social, educational and volunteer opportunities in hopes of bringing people out of isolation and empower them with a sense of confidence and self esteem as well a sense of power and control over their future and quality of life in a safe, non-judgmental environment.
Services Offered
• Information & Referral - Provides information and referrals to other service providers to help meet specific needs of clients when these services are not provided by North County Health Services (NCHS).
• Peer Advocacy - Provides one-on-one information, referrals, advocacy and emotional support for people living with HIV/AIDS. This is available to all people infected or affected by HIV/AIDS. Many times this is the first contact for those with HIV/AIDS before needing complete case management. Client who have case managers are also served.• Bi-Lingual Services - All of the provided services are also available in Spanish.
• Access to Primary Medical Care - As a program of NCHS, we are able to provide direct access to HIV specialists as well as experienced doctors and nurses for people who are newly diagnosed or require a primary care provider in North County.
• HIV Testing - Ora-Quick HIV testing is available at no charge by a certified testing and counseling staff.
• Case Management - Depending on the level of personal need and illness, a person infected with HIV/AIDS may require the services of a case manager. Case managers provide personalized assistance with social, medical and treatment services as well as financial benefits eligibility.
• ADAP Enrollment - The AIDS Drug Assistance Program (ADAP) provides free HIV/AIDS related drugs to low income people who meet the qualifications.
• Mental Health Counseling - Individual, couples, group and family counseling for those with issues regarding HIV/AIDS and every day life concerns. There is also individual women and children’s counseling for those infected and affected. These services are provided in collaboration with The Center, Christie’s Place and UCSD for eligible clients.
• Women/ Children/Family Advocate - Provided through collaboration with Christie’s Place, a family advocate provides one-on-one information, referrals and advocacy specifically for women, children and families.
• Legal Clinic - A collaboration with The San Diego Volunteer Lawyer Program provides legal services to the HIV/AIDS community. The clinic provides advice and/or assistance on a variety of legal issues for eligible clients.
• Educational Presentations - A variety of guest speakers from medical doctors, nutritionists and pharmacists cover topics of special interest to the HIV/AIDS community, such as treatment, drug trials, nutrition, etc.
• Social Events - A variety of special events are offered to encourage positive attitudes, provide mutual social support and help improve the quality of life for those infected and affected by HIV/AIDS.
• Volunteer Opportunities - Volunteer positions are available for people infected or affected by HIV/AIDS, empowering them by giving back to their community.
