• To ensure that all our patients receive the right to privacy, security and respect as well as respect for their personal values and beliefs, regardless of race, color, creed, sex or personal preferences and have:.
• The right to be treated with respect.
• The right to feel safe and secure in our health center.
• The right to privacy during interviews or during exams.
• The right to privacy of your medical records.
• The right to know the name of your health care provider.
• The right to know the training level of all people caring for you.
• The right to understand your health problem, tests and treatments and their costs.
• The right to ask questions when you don’t understand.
• The right to give your consent for test or treatment.
• The right to discuss your treatment and tests with your family members as you wish.
• The right to be informed of unanticipated outcomes and when appropriate be explained to family members.
• The right to confidentiality in all matters.
• The right to expect staff to help you with your needs whenever possible.
• The right to know the agencies use and disclosure of your personal health information.
• The right to request in writing an amendment to your personal medical record.
• The right to inspect and copy your medical record.
• The right to request and receive an accounting of all disclosures.
• The right to receive our Notice of Privacy Practices.
