Project Summary
The Salud Project is a culturally and linguistically tailored, evidence-based practice (EBP) that includes the Diabetes Self-Management Program training curricula developed by Stanford University. The purpose is to increase self-care by educating patients in the knowledge and skills needed to take control of and manage their diabetes; prevention and reduction of depression and its symptoms; prevention of and/or reduction in emergency room visits; client satisfaction in services; and in increasing access to care.

Target Audience
Hispanic adults 60 years and older, NCHS registered and active patients, diagnosed with diabetes and depression or at risk of developing it due to physical chronic illness.

County of San Diego Health and Human Services Agency (HHSA), in collaboration with UCSD School of Medicine.
