What information is in the NCHS Electronic Health Record (EHR)?
An electronic health record is a computer-based record that will hold all of your NCHS medical information. Personal information such as name, birth date, phone number and address will be stored in this record. All of your NCHS medical history will also be a part of your EHR. More importantly, medical staff will be able to view important information such as reasons for and results from past visits, lab results, medications, x-ray reports and any notes that your medical providers have added with the simple click of a button!

EHR will help your NCHS medical team do their job better and faster. Your medical providers will have correct and complete information about your health. Having access to information quickly will allow them to give you the best care possible during routine and emergency visits.
